while trying to cancel my vodafone DSL contract using the form it asks for " customer or subscriber password" which i did not find anywhere on my contract.
can somebody please help me here. thanks

Sorry, but the fact that you already have an internet connection at the new apartment is no valid reason for a premature cancellation of your cable internet contract.
You can only cancel your [landline] contract prematurely by law in the case of moving if the provider is not able to provide you the same kind of service that you had before.
This means, that a cancellation is possible in the following cases:
a) cable internet by Vodafone is not possible at all [no cable connection to the house, other cable provider]
b) cable internet by Vodafone is possible, but not with the booked speed [e.g. you had a 400 MBit/s contract and only 200 MBit/s are available at the new address]
In these cases, you are allowed by law to prematurely end the contract with a 3 months period from the day of moving (or the day that the cancellation letter is received by Vodafone - the later counts).
Vodafone makes one other case possible:
If moving together -AND- two Vodafone internet contracts exist, Vodafone will end the lower of both contracts - but this is only valid for two Vodafone contracts and if moving together!

have you reported a move? Is cable Internet possible at the new address? What have you done so far? From which provider is the contract in the new apartment? We need more information to give you some advice.
Maybe you had the wrong form. No password is actually required, just the customer number. Please note that you are a cable customer and do not have DSL.

Hi Grit,
There is already and internet connection available at my new apartment by the landlord's name. so there is no need for me to report a move. i want to cancel it because i don't need it anymore at my new place. please let me know if you want a proof of my new address and internet connection i will send it over.
NOTE. Also my 2 years contract will end in Aug 2018 and i don't want to extend my contract anymore. so please do let me know what i need to do to cancel my contract. thanks

Also i used this form for termination of the contract online. where it asks for (Customer or subscriber password: *) which i don't know.

it's not as easy as you make it for yourself. If we can supply at the new address, the contract will move. Answer the questions, too, please. From which provider is the contract at the new address? The move should be reported in any case.

The contract at the new address is from 1&1 and its in the name of the apartment owner.
Ok if its not possible to cancel this way. then i would like to not extend my contract further as my 2 years contract will end in Aug 2018. I dont want to extend it anymore afterwards. please let me know what can i do for this so my contract will not be extended to the next year? thanks

It might be that you even have missed the latest possible time for cancelling your contract regularly (3 months in advance to the ending date) - so it might have already renewed for a further year [as agreed in the contract].
And the contract that the apartment owner has is no valid point to cancel your "old" contract...

As my 2 years contract will end by the end of Aug 2018. I dont want to extend it any further. can you please cancel my contract for the next possible date so its not extended to the next year?
