Internet keeps disconnecting for over 3 weeks now.

Hello, For over 3 weeks now my internet keeps disconnecting frequently, 10-15 minutes or so before it comes back a few minutes later. I have called over 10 times and no one seems to be able to help me. 

Can someone please help me resolve this issue? This is affecting my work life.
