Moving back to home country and would like to cancel contract

The question is:

Dear Sir/Madam,

I have a new job so am moving from Germany back to the Netherlands. My Employment and rental contract for the apartment end on the 30th of April. I would also like to make this the last day for my internet connection.

I have attached the rental contract for the house I am moving to in the Netherlands. (Note: My family already lives there. They did not move to Germany)

I worked the resignation. Please note that the cancellation will take place 3 months after the move date.  This is regulated by the Telecommunications Act. A written confirmation is on the way to you. Do you have any further questions?
