Wifi Hotspot Flat cancelation

I haven't been successful try to use the contact form to cancel my Wifi Hotspot Flat service.

1. There is no "Hotspot" selection.

2. Being instructed to use "Mobile", but I don't have a Vodafone mobile number

3. Under "DSL", the form won't submit and complains of my form being incorrect.

Can someone please help me cancel my service?
