Moving house in Germany

Hello,

I am moving from Dresden to a new house in Brunnthal at end of July. And want to move my internet, I apply online. I am not sure if I still need to do anything like termination on current address. I can't get the support via phone as my german is not so good.

Please call me to confirm what I do is correct and no problem my internet to the new place.
