Closing account- English speaking

Hi,

I will be leaving the country in the next few months and keen to understand the process for closing my account. I don't have an exact leaving date yet.

How much notice do I need to give? 

Is there a form to fill in?

When will I receive my last statement? How would you work out if you owe me $ or I owe you? My current statements are not exact
