how to transfer a contract

I'm a tenant of a flat and have a DSL contract opened by myself 2 and half years ago. my landload would like to take over my contract, how should we do?

you just need the InfoDok 297, fill it all in and send it back to vodafone.

I now have a further question.

after filling the Documment-297, I noticed there are two vodafone addresses.

one is on first page: it's 40839 Ratingen

the other one is on page 5, it's 40875 Ratingen.

which address should I send to?

after sending out the document, I got an auto reply from vodafone said that you need to have a copy of the new contractor's ID, how can we send the copy? is there exist an email we can send the copy to?
