Termination of contract

Hello...

I moved out of Germany back to my home country.. so I cancelled my contract and received the concellation email and Now I have to send back device to Vodafone

So, the confusion I have return label from start of the contract..  Should I have to use it?... because the return address on the return label is Am tower 222 and the address I received in the cancellation emial is AM tower 250..

So which address should I use?

And do i have to pay the return dhl charges.?  and what other documents I have to send back to vodafone.
