Moving out of Germany

Hi,

Currently I have a cable internet contract that normally ends in May 2020. But I am moving out of Germany much earlier than I was expecting. I will be leaving at the end of January, and I would like to cancel my internet accordingly.

Can you please help me with the steps?

And also to which address should I send the modem towards the end of January?
