I lost the paper that comes with my sim card. I need a password in order add my cell in our account online. How do I get that password?

This is prepaid (CallYa) sim. We cannot find the envelope/papers that came with the sim.

Getting a new customer password please download this form --> InfoDok 265 <--, fill out your data and send the form to that email address: customerservice.o@vodafone.com

It takes around 2-3 days to set up a new customer password. You'll get an SMS after completing.
