Moving from Essen to Greifswald

I have vodafone (unitymedia) eazy 20 contract in Essen and now from september 1st I am moving to Greifswald. Therefore I wan to know, how can i switch my existing contract to new adress and if this is not possible then how can I cancel my existing contract.
