Contract Cancellation

I have a DSL contract, I am moving abroad at the end of December and I want to cancel my contract. 

Who should I contact? English is preferred.

My colleagues need the reason for the termination and the exact date, when you are leaving Germany.

They will then get back to you via e-mail.
