Completing form #500 914 1020 Transfering Callya contract to a new person, what is the Kundenkonto?

I'm moving my phone number/(Callya account) from my girlfriend's account to my own Name/account. On the transfer form #500 914 1020, there is a checkbox for "Übertrag auf schon bestehendes Kundenkonto" with a space underneath. Can I enter my gigcube account Kunden-Nr there so the phone number/(Call ya account) gets added to the existing account in my name or should I leave that checkbox and space both empty? Thank you for any advice you can share!
