Change contract to friend name

Hello,

I am using Vodafone internet 100mbps at the house. now I am moving to another city. so I have decided to transfer my contract to my friend's name. I already fill the foam for the transfer contract with the signature. but I do not know where to send this foam. i try so much time to talk with customer care but they don't speak english. please let me know as soon as possible, where to send I this foam and what other document need?
