Cancelling contract due to moving out of Germany

I am moving out of Germany on the 28th of this month (September) and need to quickly cancel my contract. I have a few questions:

What is the correct channel to request my cancellation (preferably with english speaking support)?
Do I need to fill out a specific form or would a regular letter explaining the situation suffice?
Do I need to present my apartment de-registration with the city office or will simply providing a picture of my plane ticket and student visa abroad suffice?
Where do I send my router to and do I need to pay for this?
Will I still be charged for three months before the cancellation takes effect? (I was not informed of a notice period when signing the contract - in fact I was only told I could cancel prematurely because I intended to leave the country)
